Emergency Student Information Form
OCPS requires a new emergency student information form for all students every school year. Please complete the current form and return it to the school as soon as possible. Please list multiple adults who can be contacted in case of an emergency. If an early pick-up is needed, only those individuals listed on the form will be permitted to pick up your child from school. Please inform the school of any changes or updates.

Medical Notes
If your child is absent from school, and you have a medical excuse note, please give it to the attendance clerk in the office. If your child has a doctor’s note with specific instructions (e.g., no PE/sports, must wear brace, use crutches, etc.), please give it to the school nurse, and he will notify the teachers.
Sports Physicals
Students interested in playing on a school-sponsored sports team (soccer, volleyball, basketball, or track) must have an up-to-date sports physical on file before tryouts. Sports physicals are valid for one year. Please contact Coach Foster at Todd.Foster@ocps.net for forms or questions.
Immunizations
All students must be up-to-date with their immunizations. Students entering 7th grade may need a Tdap booster, which must be recorded on an updated DH680 form from the physician. Please talk to your health care provider about whether your child is up-to-date with immunizations. The immunization form should be submitted to the main office for processing.
Medication at School
The school nurse can administer prescription and over-the-counter medications during the school day, if needed. The parent/guardian must bring the medication to the clinic in the original pharmacy bottle with the label attached. If you would like to keep over-the-counter medication in the clinic (e.g., Benadryl, eye drops, TUMS, etc.), the parent must bring in an unopened bottle. Please complete the medication packet from below and bring it to the clinic along with the medication. The clinic does not have stock medications to administer to students. Students may not bring medication to school or carry it in their backpacks, except for Tylenol/Advil.
**Please note: In accordance with OCPS policy, if students are found with medication or unauthorized inhalers, EpiPens, etc., the items will be taken and the parent/guardian will need to come to the school to pick up the items.
Students with Asthma and Inhalers
If your child has asthma, you can store an inhaler in the clinic. If you would like your child to self-carry an inhaler in his/her backpack, you must complete the Self-Carry form and have it signed by your family’s health care provider. Students are not permitted to self-carry an inhaler without a completed form. Please obtain a current asthma action plan from your provider.
Students with Severe Allergies and EpiPens
If your child has severe allergies and requires an epinephrine injector (e.g., EpiPen, Auvi-Q, etc.), you can store it in the clinic. Students may also self-carry an injector in their backpack as long as the appropriate paperwork is completed. Several forms must be completed by the parent/guardian and the health care provider. Please make an appointment with your health care provider to have the forms completed and signed as soon as possible.

Students with Diabetes and Diabetes Testing Supplies
If your child is diabetic and needs to self-carry testing supplies and insulin, several forms are required from the health care provider. Please make an appointment with the health care provider to obtain a current copy of the Diabetes Medical Management Plan and for the appropriate signatures on the Self-Carry form.

If your child has other special medical needs, please contact the school nurse so that we can discuss how to best meet those needs.
